Care Managers Connect Families with Quality Resources

The care management program links individuals and families with services that help individuals age 60 and older remain independent and in their homes.

Care managers maintain a directory of resources that are available for the elderly and their caregivers, including social service agencies, ministeral alliances, and state agencies that provide resources and assistance.

Care managers also consult with clients and families about physical, social, and economic needs. Together, a plan of care for both needed and wanted services is developed and referrals to appropriate agencies, organizations, or individuals are made.

Care Managers monitor services provided to the client to assure that quality and ongoing services are administered appropriately.
