EnhanceWellness Takes a Holistic Approach to
Health and Wellness
EnhanceWellness, conducted by a trained nurse or social worker, is an individualized, community-based wellness intervention program that is designed to improve the health and functioning of older adults.

Program goals include identifying risk factors for disability and promoting behavioral changes to reverse these factors. Additional desired outcomes include:
  • Decreased hospital stays
  • Decreased use of psychoactive drugs
  • Decreased depression
  • Increased physical activity
  • Increased confidence in managing one’s health
To meet these outcomes, the program contains three stages:

Screen. Participants complete a detailed questionnaire that includes a variety of health measures like confidence to exercise, manage health issues, and work with a physician.

Other factors screened include socialization, depression and anxiety, weight and nutrition, fall history and risk, smoking and alcohol, memory and medication use.

Plan. EnhanceWellness staff work with the participant to develop a health action plan. The plan focuses on at risk health issues the participant chooses to focus on.

Action. The participant moves into action with the support of the EnhanceWellness team and/or a trained volunteer mentor. They help with problem-solving, health education and referrals to support groups for additional services.

At the end of a six-month period, the participant completes another questionnaire which is compared to the original one. Immediate feedback is offered regarding the participant’s success. Afterwards, the participant may choose to continue with the program or graduate. Reported outcomes among program participants include:
  • 80% maintain or improve in the area they focused on
  • 72% decrease in hospital stays
  • 35% decrease in the use of psychoactive medicine
  • 11% decrease in depression
