The National Family Caregiver Program, accessed through Care Connection, is designed to help you locate invaluable resources to make your role as a caregiver easier and less stressful. At your request, our Care Managers will provide the following information:

Available resources in your area.

Assistance in gaining access to services, including care management.

Training to assist you in making decisions and solving problems in your role as a caregiver.

Respite care for temporary relief from your caregiving responsibilities.

Supplemental services, which are provided on a limited basis, to complement the care given by caregivers. These services are intended to meet caregiver needs not covered by other areas of the program. This may include: incontinence and other supplies, minor home modifications, nutritional supplements and other supplies.
Individuals available for these services include:

Caregivers of older adults ages 60 and over;

Grandparents and relative caregivers (ages 55 and older) of children under age 18; and
-
Senior caregivers 60+ who are sole caregivers of children and individuals with down syndrome or who have developmental disabilities.
