![]() Live Well by Taking Control of Your Health Chronic diseases pose a growing threat to aging individuals, health care providers and those who cover health care costs. In the coming years, the number of mature adults battling chronic illness and disability is expected to escalate – resulting in pain and suffering for these individuals, while increasing the demand on health and long-term care systems. There’s power in mobility. Regular physical activity can decrease incidences of high blood pressure, osteoporosis, heart disease, diabetes and depression. Seniors, including the frailest, can improve their health and maintain their independence by incorporating daily physical activity. Care Connection offers several evidence-based Living Well programs aimed at empowering seniors to take control of their health: Chronic Disease Self-management Falls Prevention (Matter of Balance) Fitness Programs EnhanceWellness Laughter Therapy Over the past years, these classes have been highly regarded by participants who have reported reduced pain and overall increased activity. Matter of Balance class participants have reported a significant reduction in the number of falls, coupled with a reduced fear of falling. These evidence-based programs have gone through the equivalent of clinical trials. Class coaches are extensively trained and must receive at least one follow up visit from a Master Trainer to ensure fidelity with the program.
