Care manager Laura Miles will present information and tools to help older adults and their caregivers with advance care planning. The presentation is in observation of National Healthcare Decisions Day. The session will be free and open to the public. The presenter will guide participants through “Five Wishes,” advance healthcare directive, which would be considered a legal document once it’s signed and notarized. Copies of another publication, “I Have Put My House in Order,” also will be distributed. This booklet guides the user in providing important information about his home, family, estate planning, funeral plans, finances, and life insurance.
Both the “Five Wishes” and the “I Have Put My House in Order” documents will be given free to workshop participants.
